Before undergoing epilepsy surgery, magnetic resonance spectroscopy (MRS) can play a valuable role in predicting outcomes. A meta-analysis conducted by Willmann and colleagues at the University Hospital in Mannheim, Germany, highlights the potential of 1H-MRS in identifying epileptogenic foci prior to surgical intervention. The study was published online on August 3, 2006, in *Epilepsy Research*.

The analysis included 22 studies published between 1992 and 2003, all focusing on patients with refractory temporal lobe epilepsy. However, the studies varied significantly in methodology, technical approaches, and data interpretation. Despite this heterogeneity, the findings suggest that MRS can provide meaningful insights into patient outcomes.

According to the results, 64% of patients showed ipsilateral MRS abnormalities corresponding to the epileptogenic focus, while 72% of those who had good postoperative outcomes also exhibited similar findings. The positive predictive value for a good outcome based on ipsilateral MRS abnormalities was 82%. Moreover, patients with unilateral MRS abnormalities were more likely to have better seizure control after surgery compared to those with bilateral abnormalities, with an odds ratio of 4.891.

The researchers concluded that although MRS is still primarily a research tool, it shows promising clinical potential. When MRS detects abnormalities consistent with the epileptogenic focus, it correlates with improved post-surgical outcomes. However, the predictive accuracy may vary depending on the patient population. Therefore, further prospective studies are recommended, particularly in patients with MRI-negative or EEG-characterized non-localizable epilepsy.
